Hilltop Community Resources’ B4 Babies & Beyond is an essential program for pregnant women and families with children under five, offering services to ensure healthy pregnancies and strong starts. In Mesa and Montrose counties, B4 Babies helps with prenatal care, insurance enrollment, nutrition counseling, parenting education, and resource coordination. They focus on supporting both uninsured and underinsured families. Additionally, their Baby and Me Tobacco Free program promotes family wellness by helping parents quit smoking and offering incentives like free diapers for up to a year. It’s all about supporting families every step of the way!
A Unique Care Model
One of the standout features of Hilltop’s B4 Babies program is the help it provides with Medicaid and CHP+ enrollment, making sure families have access to affordable healthcare services like prenatal care, mental health support and doctor visits. They even offer transportation to appointments, and services are available in both English and Spanish, ensuring greater access for everyone.
Hilltop takes a flexible, community-driven approach, meeting families where they feel most comfortable—whether it’s at home, in a park or the office. This participant-led model allows mothers to set the pace, making it easier for them to engage with the support they need while managing the demands of motherhood. The program also specializes in helping mothers struggling with substance use disorders (SUD), connecting them to expert care and supporting them throughout their recovery journey.
One success story from the program highlights a mother who, with the guidance of her family navigator, achieved her goals and now works full-time and lives independently. “I would not be where I am if it was not for her,” she shared, highlighting Hilltop’s empowering, non-judgmental approach.
Hilltop’s commitment to flexibility and personalized care ensures that families across Mesa and Montrose counties have access to the resources they need to thrive. By meeting families where they are—both physically and emotionally—Hilltop continues to make a lasting impact on the communities it serves.

What Would Want Other Providers to Know?
To improve participant engagement and family outcomes, providers should adopt a flexible and holistic approach to case management. This means meeting clients where they are—whether that’s physically, emotionally or in terms of their individual needs. By tailoring support to the unique circumstances of each family and allowing them to set the pace, providers create a more empathetic and supportive environment. We have found this personalized, non-judgmental approach fosters trust and encourages clients to stay engaged, ultimately leading to more successful outcomes for families.
